agilon and Eagle Physicians’ combined expertise will expand value-based care to more seniors across North Carolina and accelerate care transformation
AUSTIN, TX, September 18, 2024 – agilon health (NYSE: AGL), the trusted partner empowering physicians to transform health care in our communities, today announced that it has formed a long-term partnership with Eagle Physicians, an independent, multi-specialty group practice in North Carolina. With this partnership, agilon will enable Eagle Physicians’ transition to a full-risk, value-based care model to enhance outcomes and quality of care for their senior patients beginning in 2025.
Eagle Physicians joins agilon’s growing network of physician partners and will be part of the 2025 class of new partner organizations. Announced in May 2024, the other class of 2025 partners include Graves Gilbert Clinic in Kentucky, Mankato Clinic in Minnesota, Springfield Clinic in Illinois, and Twin Cities Network in Minnesota. With these new partnerships, agilon’s Physician Network will grow to include over 3,000 primary care physicians (PCPs) who are delivering value-based care to more than 700,000 senior patients in 30+ communities.

Today, more than 30 physician groups and health systems are in long-term partnerships with agilon and have transitioned to full-risk for their Medicare patients through agilon’s technology-enabled platform.
