Summa Health Hospital Designates Entire Floor for Pioneer Physicians and Paradigm Senior Care Advantage Patients
In partnership with Pioneer Physicians Network, Summa Health is designating an entire floor of City Hospital to provide quality care and special treatment for Pioneer’s patients. This new arrangement will allow for better continuity of care for these patients – especially those with chronic health conditions.
With this new partnership comes the convenience of having telemetry devices, heart-monitoring and respiratory equipment all on one floor. It also means that Pioneer patients have immediate access to hospital specialists. In addition, a medical team consisting of health care managers, nurses and rounding physicians from both Pioneer and Summa Health work in collaboration to treat patients and get them back on their feet again and avoid readmission. Typically, patients are spread throughout the hospital and taken care of by many unrelated care teams that don’t know each other well. “A team that knows each other well will perform much better.” Gary Pinta, MD, President of Pioneer Physicians Network, is excited that the new arrangement also provides greater efficiency and improved communication between the medical teams. He admits that the new “small hospital within a large hospital” is already resulting in better outcomes for Medicare patients.
